WORK PACKAGE 7 OBJECTIVE

WP7 aims to deliver an integrated, person-centred digital ecosystem that empowers clinicians and ageing individuals through personalised risk prediction, continuous lifestyle monitoring, and adaptive health interventions. By combining AI-driven decision support tools, mobile-based data collection, and behaviour-change coaching, WP7 enables proactive care planning, early prevention, and improved self-management of age-related conditions; all seamlessly connected via secure, standards-based platforms.

IMPORTANT?

STAGE is focused on delivering tangible benefits for citizens across the EU.

By developing tools built on the research conducted in STAGE, we create opportunities for clinicians and patients across the EU to target interventions earlier.

These technology-focused solutions will help meet the diagnostic needs of Europe’s ageing population whilst encouraging healthy ageing.

WP7 Activities and Interlinkages

WP7 will draw on work from across the STAGE project. It will use the predictive algorithms and AI models developed by WP6, the work on multi-morbidity according to neighbourhoods conducted by WP3, the wider exposome predictors outlined in WP4, and molecular hallmarks of ageing identified in WP5. It will also work extensively with participants of the studies involved in WP8, where the technology interventions will be tested. Key tasks of WP7 include:

T7.1

Decision Support Tools

Creating personalised support tools for community primary
health care providers. They will help screen citizens for risk factors, offer preventative measures, and help proactively manage age-related conditions, utilising the research, especially the AI models, done in other WPs.

T7.2

Health and Intervention Monitoring

Passive and active monitoring will be carried out using the Ab.Acus mobile application to collect data on mobility, cognitive functions, general well-being, and dietary factors among older adults. The health and intervention monitoring will be delivered to all participants of the Northern Finland Birth Cohorts (NFBC) 1966 study in WP8.
T7.3

Personalised Intervention

Providing a digitally delivered, evidence-based behaviour change programme to participants of the population-based KORA study in WP8, using the Liva app. The Liva intervention combines personalised, one-on-one lifestyle coaching with innovative digital technology. The app incorporates features such as goal setting, progress tracking, and educational content, and the programme aims to support people to make and sustain healthier habits.

T7.4

Integration of Applications and Solutions

Integrating person-centred applications and solutions for clinicians and ageing citizens. A system will be developed to enable seamless data exchange between the decision support tool, the health monitoring tool, and the intervention delivery platform. This work will be closely coordinated with WP9.

  • Help older citizens prevent, monitor and manage progression of age-related diseases, while promoting healthy lifestyles and ageing in place.
  • Primary and community-based health and care services will be better equipped to early identify people at risk of developing non-communicable diseases and to implement new intervention tools into these services.
